Description
What is Tesamorelin?
Tesamorelin is a stabilized synthetic analog of growth hormone–releasing hormone (GHRH) consisting of 44 amino acids with an N-terminal trans-3-hexenoic acid modification that extends its half-life. It is approved by the FDA (brand name Egrifta®) for the reduction of excess abdominal fat in HIV-infected patients with lipodystrophy and has been studied in multiple human clinical trials for visceral adipose tissue reduction and cognitive function in aging adults.
Mechanism of Action
- Selective agonism of pituitary GHRH receptors
- Stimulation of pulsatile endogenous growth hormone (GH) release
- Downstream increases in circulating IGF-1

Research-Reference Dosing
Human trial dosing from published peer-reviewed studies. Provided for research reference only.
- HIV-lipodystrophy trials: 2 mg subcutaneous once daily (Falutz et al., NEJM 2007; Stanley et al., JAMA 2014)
- Cognitive aging study: 1 mg subcutaneous daily for 20 weeks (Baker et al., Arch Neurol 2012)
- NAFLD reduction trial: 2 mg subcutaneous once daily for 12 months (Stanley et al., Lancet HIV 2019)
Research Findings (Published Human Data)
- ~15–18% reduction in visceral adipose tissue over 26 weeks in HIV-lipodystrophy trials
- Significant reduction in liver fat fraction in NAFLD studies
- Improvements in executive function and verbal memory in healthy older adults and those with mild cognitive impairment
- Increased IGF-1 within physiologic range
Known Side Effects Reported in Trials
- Common: injection-site redness/itching, arthralgia, myalgia, peripheral edema
- Less common: glucose intolerance/hyperglycemia, carpal tunnel symptoms, headache
- Serious/rare: hypersensitivity reactions; caution with active malignancy or pituitary tumors
